State honors local nonprofit charitable clinic for outstanding partnership advancing community health in Hendricks County.
AVON, Ind. - IndianaStop -- Hope Healthcare Services is honored to receive the Indiana Department of Health's Public Health Partnership Award, presented in recognition of "Outstanding Partnership to Advance Community Health."
The award recognizes Hope's collaboration with the Hendricks County Health Department—enhanced by Health First Indiana support—to expand access to essential health services, remove barriers to care, and strengthen public health capacity for neighbors most in need.
According to the award citation, this partnership has expanded screenings, referrals, care coordination, dental access, and modern health infrastructure—creating lasting improvements and meaningful opportunities for better health for all.
"We're grateful to the Indiana Department of Health for this recognition, and especially thankful for the partnership with the Hendricks County Health Department," said Cliff Sweazey, Executive Director of Hope Healthcare Services. "This award reflects what happens when community partners work together to achieve a common goal. We believe everyone deserves access to affordable, compassionate, holistic care, and we serve a population that is often overlooked—individuals and families without insurance."

The award was signed by Indiana State Health Commissioner Lindsay M. Weaver, M.D., F.A.C.E.P., and Megan Lytle, Assistant Commissioner for Public Health Protection.
